Scheduling Your Appointment

Once you make an appointment at our office, that time will be reserved for you. If, for some reason, you are unable to keep your scheduled appointment time, call us as soon as possible to reschedule.

What to Bring to Your Appointment

To provide you with prompt dental treatment, please be sure to bring the following when you visit our office:

Medical & dental insurance information/card
Photo identification
Any medical records about your visit

Please arrive 15 minutes before your scheduled appointment time to fill out the necessary paperwork. If you would like to complete your paperwork before your appointment, please click on the link below.

New Patients

Insurance

We accept most dental and medical PPO insurance plans. Please bring your insurance information with you to your first appointment. Our office staff will file any necessary claims and work to get you the maximum amount of benefits allowed by your plan. If you have any questions about your insurance plan, feel free to call us so we can help you.

Payment

Payment is expected at the time services are rendered, unless other arrangements have been made. This includes any copayments or deductibles (amount not covered by your insurance).

For your convenience, our office accepts cash, Visa, MasterCard, Discover & CareCredit.

Please Note:

We alternate between Wednesdays and Fridays. If we are open on a Wednesday, we will be closed on the following Friday, and vice versa.
We are only open on Saturdays twice a month.
